Important Conditions for Using Health Check-Up Services

1. This health check-up package is suitable only for outpatients (OPD) who have received prior consultation and for individuals who can take care of themselves.
2. If the package includes a heart examination with an Exercise Stress Test (EST), the patient must be physically fit enough to run on a treadmill.
3. For safety and accurate results, you should refrain from eating, smoking, and consuming caffeinated beverages for at least 8-12 hours before the examination. Additionally, ensure you get adequate sleep and avoid heavy exercise on the day of the service.
4. The hospital does not allow changes to the items in the package or substitutions with other packages.
5. This price does not include fees for specialist doctors, radiology tests, additional medical treatments, or medications prescribed by the doctor.
6. The hospital reserves the right to change the service conditions without prior notice.
7. Please present the coupon before receiving services. The hospital does not allow the coupon to be exchanged for cash.
8. After paying for the health check-up package, please make an appointment at least 3 days in advance via email at Nuntapak_sut@Phyathai.com to confirm your service. Otherwise, the hospital reserves the right to refuse service.
